- Previous fluoroquinolone tendinopathy; epilepsy; myasthenia gravis.

Legacy FDA category C. Avoid in pregnancy when a safer alternative exists.

Clinical notes
Fluoroquinolone cautions as for ciprofloxacin. Many Indian products combine ofloxacin with ornidazole — check every component on the label.

200–400 mg — Every 12 hours (max 400 mg per dose, 800 mg per day).
