Chickenpox (varicella) — when treatment is indicated

20 mg/kg/dose · four times daily · max 800 mg/dose, 3200 mg/day
Duration: 5 days
Eligibility: ≥2 years
Consider for adolescents ≥13 years, chronic skin or lung disease, long-term salicylate or steroid therapy, or secondary household cases; start within 24 hours of the rash. Immunocompromised children need IV treatment in hospital.

Herpes simplex gingivostomatitis / mucocutaneous HSV

<2 years: 100 mg per dose · five times daily
Duration: 5 days (longer if new lesions appear or healing is incomplete)
≥2 years: 200 mg per dose · five times daily
Duration: 5 days (longer if new lesions appear or healing is incomplete)
Eligibility: ≥3 months
Most useful when started within 72 hours of onset. Infants under 3 months with suspected HSV need IV aciclovir in hospital.

Neonatal herpes simplex (suspected or confirmed) — IV Hospital

20 mg/kg/dose · every 8 hours, infused over 1 hour
Duration: 14 days (skin, eye, mouth) or at least 21 days (CNS or disseminated disease) · Reduce the dose interval in renal impairment. Monitor renal function and neutrophil count.

Clinical notes

Oral bioavailability is low (15–30%) — oral treatment is not adequate for encephalitis, neonatal or disseminated HSV, or varicella in immunocompromised children.

Adult and adolescent (≥40 kg) dose

Herpes simplex: 400 mg three times daily; varicella/zoster: 800 mg five times daily — Five times daily for 7 days (varicella/zoster) (max 800 mg per dose).
